Can not see fields name URL email in Guestbook

I created guestbook by manage extras/add extra/guest book and then saved.
When I go to see/fill in questbook, there is no field of name URL email even I activited them before saved. Any advice???

When you are online, you can NOT see these options in your guestbook page!
Add guestbook extra again, then edit the options, and then save the page! Lastly after saving the page, click on 'logout' button. After logout, you can see the options, because you are offline here. Shorty, Offline users and visitors can see these options, online users can not see ..
